I think Washii has a point. Also, it makes sense Mio replaced her surname with Ritsu's surname as she is the "man" of the relationship. Mio probably gave birth to Kanade.

That's not the view of it in Japan.
Over there, the one to take the other's name is determined by social/financial standing. The one of lesser standing gets adopted into the family of greater standing. This can go either way; if the man was of a lesser family, he takes the bride's surname.
