What KChasm said. HHNNNGGG has basically replaced "SO MOE I'M GONNA DIE!" and "I'M GONNA TAKE YOU HOME" or "YOU GONNA GET LOVED TENDERLY" etc. as standard stock responses to overwhelmingly cute characters.
The nutbladder thing is actually, specifically, with regards to Hidamari Sketch. According to LurkMore, an /a/ thread had someone who claimed he was so turned on by the show's "wide head" style that he masturbated until he "broke his nutbladder" (presumably, his testicles). The odd wording and subject matter made it a meme.

Also, it might have something to do with taking Raita's concept art and running with it without asking permission, but I doubt that's anywhere near all of it.

While I don't speak moonrunes, I've heard a number of stories of any attempt by Westerners to post on boards like 2ch and Futaba ending in basically "gaijin go home." So, yeah, I'd say there is a good deal of xenophobia in my opinion.

Language barrier, perhaps? There are plenty of groups doing Japanese -> English translations for visual novels and whatnot but I've never heard of it going the other way around.

I visited a 2ch thread about Katawa Shoujo once. The main complaint was that "katawa" is an offensive term. The runner-up was that getting off to deformities is creepy.
Now that you've mentioned it, it does seem rather offensive and creepy. I guess I'll just stick to regular loli tentacle guro with a side dish of traps and futanari.
The real thing is that japanese doesnt really like to touch terms about discapacitated people, so that's why they are biased about this game (and that doesnt let them see how good this game is). They're like taboo. For what I've heard of a friend, they seem to evade looking at them, or making it seem like they don't exist. Dont ask me why, but if they've done this forever, its like tradition. One tradition that should never exist. Like hding the fact you have a hikikomori in ur house, instead of helping him, just becasue it's bad for the image of the family (THAT'S ENTIRELY TRUE!).

As I understand it, "Katawa" literally means "flawed" or "imperfect". I think the idea the creators had was to take the word in stride, like "otaku" or "gaijin", which I believe are more loaded terms in Japan. I'm willing to wager money that there's never been a story similar to "Katawa Shoujo" that really took off in Japan. That is, a series that has characters with disabilities, but an honest, sweet one, *not* one where that characters in question are treated *only* like fetish objects.
