It's not a twist in a plot, it's not something that happens to the character. She is introduced that way right from the beginning.

That's true. It's quite hard for it to be a spoiler when it's revealed in less than 5 minutes into the first episode.

It does look like that. Whoever was drawing it obviously forgot that the pedal arms are always at 180 degrees to each other.
