I knew the extreme makeover made him forget her!

And comparing her current looks to her former one it's kinda obvious why he wouldn't recognize her.

She went from long haired girl who was all smiles and seems happy and energetic to short haired and perpetually tired with a somewhat deadpan expression most of the time.

Now he just really needs to remember that dream again to get it but since it's a dream it would probably slip his mind for a while.
