In the anime series Higurashi no Naku Koro Ni, the "punishment" when the main character loses the first game is drawing on the face with the marker; the second "punishment" is injection of a chemical that makes him claw his throat out.
I have heard rumors that the needle contains some kind of horomone that causes a large number of cicadas to follow you and collect around your throat causing you lose your mind from the unceasing noise that cicadas make (hence the name ひぐらしのなく頃に translating to "When Cicadas Cry") and the swarming on your neck causing you to incoherantly claw your throat out . But then again, we'll just have to wait for the 1st answer arc to be released.

in the first story, keiichi became really crazy because of hinamizawa syndrome and hallucinated the marker that the girls had was a syringe with poison to kill him.
