A cultural difference. In Japan there's a specific part of their penal code that forbids the sharing of "indecent materials". I believe a manga artist tried challenging it in their highest court and got smacked down hard. So most distributors self-censor to not give the law an excuse since their stance has been officially demonstrated already.
All the weird and sometimes kind of gross kinks and fetishes that exists in both JAV and hentai yet the Japanese government still has a stick up it's ass over removing some mosaics.
