The operation Naoto's Shadow was talking about was to turn her into an adult, it didn't have anything to do with sexual identification..
Naoto dresses like a man purely out of practical considerations so as to be taken more seriously by adults. Naoto isn't trans.

It definitely has something to do with it, since I’m pretty sure they find out she’s a girl from the shadow.

Do you remember what part they state the intention of the operation? Is it right before the boss?
