It looks like somebody is peeing into her mouth (a little too straight a trajectory for ejaculate); it just lines up with her right nipple. Anyone who agrees that it's pee is welcome to tag - I'm too lazy to be sure of myself.
The stream of fluid going into her mouth is actually coming from outside the boundaries of the image, so it's presumably someone pissing into her mouth. It's understandable to see how it could look like whatever's coming out of her nipples is somehow arcing into her mouth, but the coloring between the fluid going into her mouth is lighter than that coming out of her nipples.
