So what’s the consensus on her background? I haven’t played the game but I read some of her entries on the wiki. Is she some sort of sleeper agent or something?

If I'm not wrong, Projekt Red instills a primal fear in all Lupus/Lupo, she was taken in, raised and protected by Kal'tsit.

It's also possible that Projekt Red is a product of a secret research group intent on creating the perfect killer. There are mentions of "Grandma" which is likely the mastermind behind her old training. Kal'tsit is probably in the process of breaking apart the conditioning Projekt Red went through so that the kid's mental and emotional capacity can finally develop in a healthy manner. After all, Kal'tsit seem to treat "Grandma" as a non-friendly entity that may come to blows with Rhodes Island in the future.

From what I remember she's a Lupo who hunts other Lupo, which is why all the other wolves are terrified of her

Not Lupo, “wolves”. IIRC, the other Lupo at Rhodes Island aren’t “real wolves” according to her, meaning she was trained to hunt something else, but since they're still terrified of her it doesn't appear to make much of a difference to them.
