The first mention means the shape (the thing she's holding), the second mention means to celebrate the beginning of something.

Probably not knowing that 5 yen coins have holes in them. Also the sentence was missing a word.

It wasn't missing, it was implied by the rest of the sentence, which originally also has no directly stated verb. It could be "Placing a ring-like ¥5 coin" or "The presence of a ring-like ¥5 coin". Simply starting it with "A ring-like ¥5 coin" covers a variety of possible implied verbs. (Then again, I'd have thought using the phrase "ring-like" would have been sufficient to imply that a ¥5 coin has a hole in it, so...)

Even if you didn't know that, she's clearly holding something ring shaped in her hand, and the sentence makes perfect sense as it is, it isn't missing anything
