I do think that the perspective is fine since she is leaning back and the main focus of the picture is the attention of her lower body being censored by the milk container of the machine (and her large breasts being milked, duh~) by mean near the center of the image, also her arms are about the right size since they look like the are restrained up and behind her head, especially since most part of them are obscure by the head, nor they are too thin for her body proportions given by the artists and they also aren't the main focus of the picture. Finally by the "weird painting" idk what tries to say (looks pretty random like is not familiar to this type of fetish maybe?) or maybe is not familiar with this artist style, if is this one maybe needs to look at the other 90 pictures in the site plus the others on his twitter and pixiv to be more familiar with his style.

Not speaking as the one who flagged this, but I've always found the "this is the artist's style" to be an unconvincing defense. After all, xkcd has an art style of stick figures, but that doesn't mean we'd want to see stick figure art on Danbooru. If an artist always does things in a way that "on model" artwork gets flagged, then maybe only the "off-model" art that accidentally looks decent should stay?
(Not that this artist looks inherently terrible the way that some of the artists *cough* Nameo *cough* this defense has been used against do.)
And I can't say I agree that perspective or the fact that her arms are behind her head are enough to mitigate the massive disparity in thickness between her arms and thighs. What you say in other sentences, that it's part of this kind of fetish to purposefully deform the body, is more accurate, but also very likely why it's being flagged in the first place. Someone who doesn't share in a fetish is generally just not GOING to understand it, since that's part of what makes it a fetish, (furry being a good example,) so anything that relies upon having a fetish to appreciate is just plain likely to be contentious to start with.
