When referring to both South and North America together we refer to them as "the Americas".
American is generally understood as referring to something related to the USA. Pan-American would refer to something related to covering either all or a lot of the Americas.
North America: continent, South America: continent.
America: short for United States of America, part of the North American continent.

What else would we call ourselves? Country's name is the United States of America, most times shortened to the United States or even just States. There are of course nicknames, most common one being "yankee" which is also used as a curse, but I don't think you'd ever be able to find an American that actually felt insulted by being referred to as such, and if you did, they're weird.
