It wouldn't be surprising, considering
right after pregnancy, you'd still have that belly. And in her case, it looks like she recently had a very brutal caesarean section.
But I don't know if the artist took this into the equation.
EDIT: But I think I know what you're thinking lore-wise. The Yharnam Stone very much looks like a fetus.
