The fandom is already taking too long to make a Touhou mod of Undertale. That said, I wonder which characters could play the major roles of Undertale. Perhaps...
Frisk -> Renko Flowey -> Marisa, if this gif is to be followed. By association, and based on that
Chara/Fallen Child -> Reimu
Toriel -> Byakuren? Shinki? Someone with motherly instincts and overly protective. Hmm... Sans -> Komachi fits his lazy demeanor to a T.
But as for his actual powers, Yukari would be a better fit.
Papyrus -> Definitely Cirno. Undyne -> Momiji? She's not depicted as a large ham in fandom, but... Alphys -> Nitori (pre-Hopeless Masquerade), natch. Goes well with Momiji as Undyne too. Metatton -> Ruukoto by virtue of being a robot. But personality wise, I dunno. There isn't a character as flamboyant as him in Gensokyo, is there? Asgore -> Miko, maybe.
Those are the ones that I came up off the top of my head. I'm sure there're better associations than these ones.

I don't think it should be done at all. That game is greatly overhyped for what it is. It's a video game soup like IWBTG, but it tries to act like it's not. Touhou is even one of the contributing parts of that soup, so making a touhou mod of it would be like some kind of weird ouroboros.)
Just go play touhou mother. Virtually the same thing.
As I was passing through, I wondered if I could find some Undertale fanart on Danbooru. Not to my surprise, I found this particular .gif and catched my attention. I read the thread thoroughly and well, I think I can share a bit of what I thought now, after some hours of spotting the thread.
I saw the options to every character upside but... Maybe, and only maybe, if a Touhou Undertale mod is made... Since the very game occurs in the underground, why not making it into some sort of a whole Underworld Hell of the Blazing Flames game? Like, for example, Snowdin could be the "old hell", where Onis are running amok drinking and playing and stuff. Hotlands wouldn't change too much, yet Waterfall would be the contrasting place.
As for the NPCs... Well, maybe Toriel would be Satori, trying to help children not to venture forth into hell. I don't exactly know, however, who would be Flowey. If Marisa's SOULless vessel transformed into a flower, or Koishi, messing with the subconscious of Frisk (which would bear a resemblance to Reimu) Showing at the end of the game (route?) that she wasn't Marisa all along, but she was just playing to get her attention and turns into Koishi.
Maybe Asgore would be Utsuho, who used human SOULs to fuel the flames of hell so they rose and burnt at utmost power, so they could break the barrier.
That... Would be all I got, I guess. Ah, Undyne could be Yuugi? That's all, cya.

The game would work well as an adaptation of EoSD or even just implementing the SDM cast into it. The protagonist would be an Original Character, a lost child who has wandered into, and gotten stuck in Gensokyo. Of course, there's a way to change your FATE, but that will require beating/killing/befriending the girl who can alter it.

Flowey → Flandre (Escaped, and the first to meet Protagonist. I know a fun game! It's called catch the friendliness pellets! The more you catch, the funner the game! Ehh?! HoW WeaK! You CaN'T eVeN TaKe a FeW FRieNDLiNeSS PeLLeTS?!)
Goatmom → Rinnousuke (Rummaging through human stuffs when they come across a Youkai and shoo it away with a talisman gifted to him by a close friend; offers protagonist refuge and bids them go to the human village or stay rather than try to brave the SDM; give the man some danmaku)
Napstablook → Three Fairies (all three are trying to hide in the open when startled; they forget to use their powers. They constantly argue amongst themselves on how to deal with protagonist, and it results in hectic attacks, but a tactical retreat later to asses the situation.)
Papyrus → Marisa (Wants to take protagonist's precious thing; decides against it, and decides to "steal their heart" instead when befriended; date fails because she's still more in love with stuff and junk)
Sans → Reimu (Still lazy, less puns, more greed... perhaps greed-related puns; is more than she seems)
Dogs → ⑨-Squad HO!!! (Minus Cirno; is possible to defeat easily by stumping them with a book of puzzles and riddles used as the original weapon you hold)

Alphys → Patchouli (Nerdy bookworm who doesn't mind helping you, because you're not a book thief. Perhaps protagonist helps her out if she's fainted from anemia and she wants to repay the favor, or something. She's punishing her overly LEWD familair Koakuma by making her small, and tiny... Koakuma is wanting to rebel and attacks the protagonist for both sustanance and to force Patchouli to return her to normal.)
Muffet → Sakuya (Catching the thief who has been wandering about. Perhaps if you helped some of her fairy maids earlier in the game... like... bought some fairy doughnuts or fairy cider, she'll let up on her attack and consider you a non-threat/friend.)
Mettaton → Chibi Koakuma (Sealed power; power returns faster than it can be drained. Perhaps not even a Chibi, but like, a small demon-looking ball-like thing? Not human-looking at all. Like a ball with wings on it's back and head, and two dot-like eyes and a small tail.)
Mettaton EX → Sexy Koakuma! (Pulling her tail causes the curse she's been put under to dissipate; nullifying her defensive power, but makes her stronger and more lewd and flirtatious! She attempts to establish dominance by trying to flirt with you and make you fall into her pace. Other minor demons/familiars watch on in awe as you deny and outflirt her. The more you ignore/outdo her the more humble she appears, aka: less succubus seductress, and more cute demon-girl.)
Also: *it's a beautiful day outside. *cicada are chirping, *fairies are singing, *humans... *no... *youkai like you should be drowning in the Sanzu river.
Goatdad → Remilia (Notes how strong you are to make it as far as her, and challenges you wholeheartedly for the chance to change your fate! She encourages you not to hold back or give mercy... until she's on the brink of death, she recognizes the strength in being able to determine when to spare your foe, even if it could mean certain death. You don't really need to leave, you could stay here and-
