If they were that directly related they wouldn't still have the same family name. Maybe it's a aunt/niece relationship or adoption.

Japanese gets to choose which family name to take. Not just the father's side.

We need a granny in Touhou. Not just characters like Suwako or whatever (who are really old but look like lolis) but an actual granny with grey hair and everything. That would be nice.

Nah, we don't know anything about their relationship other than both having same family name and being in the same "club".
The number of "Usami" generations between those two depends on how far into the future Renko and Maribel timeline is set.

I thought about it for a while and the same club idea doesn't exactly fit. The Secret Sealing Club that Sumireko started is in a high school while Meribel and Renko are in college or university. Also Sumireko's club is considered an unofficial club while with Meribel and Renko seems officiated out of many similar clubs even if they are the only two members.

I'm probably forgetting something since i read Mary and Renko's stuff a while ago, but it doesn't have to be the same exact organization.
For what we know, Sumireko's original "club" died after a short while, then Renko found some of Sumireko's memories/notes/whatever about the supernatural, got thrilled about it, and decided to *resurrect* the club using the same name while also making it "official".
