I'm just wondering, is it just the artist making them like that, or is there actually a different way to arrange the dolls?

The usual arrangement are:
Emperor and Empress
Three court ladies
Five musicians
Two ministers

Like this
Official in-game version

The only different style I've read is that the left-right position of the Emperor and Empress are reversed between the old and new capitals (Kyoto and Edo).
