I didn't even recognise that was the Shoulder Fang.
From what I can tell, the punchline is that Shinnosuke gives his weapons really simple names. His gun that's a door handle? Handle Gun. Thus, all of these weapons get renamed to really simple things, but when he gets to one that HAS a simple name, Shoulder Fang, he can't figure it out.

The gun is a car door (hence the name Door Gun), the sword is the one with a steering wheel/handle/whatever other countries call it attached to it.
