This is from the old time. I think during the Fall Event or something, where if you load the game for the first time. Images that didn't yet get fully loaded gets that portrait. Sames goes to the enemy fleet.

Missing sprite is different to this. This one is a weird bug, still occurring if i recall from my newer admiral friends

You get from both drop or building, a black Ooi sprite with NO DATA on it, but you actually obtained nothing

True on before the Fall Event, but I hear some folks still gaining drops but having that "No Data" Ooi image.
