Innate time abilities, like Kaguya's, that are really extreme compared to other human's, Eirin being surprised of seeing her in Imperishable Night, unknown true name and obscure past. However, Sakuya herself didn't seem to recognize the lunarians, or gave any other hint in SSiB/CiLR (amnesia is still a possibility, though). You could say it's on the same level of ex-Rumia theories (which i believe might have been true in the original concept, then ZUN probably dropped it for some reason).
I wonder if ZUN has anything to say about things like this in Strange Creators of Outer World, since it seems it also deals with the fandom part of Touhou (gotta fill 165 pages in one way or another after all, although it probably concentrates on derivative music and games).

Either that, or she's Gensokyo's version of The World's Stand master, and no one else can see it since no one else in Gensokyo is known to have a Stand.
