Usually it does make sense though because of the "heartless" part. The protagonist doens't perform as well because he/she is usually not willing to go to extreme lengths to get missions done and stuff. Things like using forbidden technology, magic rituals, sacrificing soldiers or even paying off mercenaries or something are usually the qualities of these types of characters.
Although half of the time, they may or may not be actual enemies. In which case, if they are gonna stay an ally, they may or may not redeem themselves in the course of the story. They could also die horribly and redeem themselves in their last dying breath seeing the error of their ways.
In a more light hearted stories, these mean characters might actually stay mean for the sake of comedy and the protagonist doesn't mind "not being the best" since being the best actually implies having a crapton of responsibility and they probably like to slak off anyway.
