They're cousins, as I recall. I think it's mentioned in SSiB, or one of the other background book things.

They're not, actually. The only relation the sisters have to anyone else we've seen so far is Eirin, and only through marriage to 2 of Eirin's grand-nephews. Whether they're still alive or not is unknown, but since we didn't see them at all in SSiB or Inaba (which were made after CiLR where the marriage relation was stated), they're probably not.

ZUN said he wants to do more material with the sisters at some point so hopefully we'll get a straight answer about the matter then. Personally I'd be satisfied with a casual manga focused just on the sisters and their possible antics on the moon or in Gensokyo, but more serious material is fine, too.
