Apparently, it's a reference to Mother 3. I don't know all of the details, but apparently you enter Tanetane Island barely alive with the only nearby healing items being psychedelic mushrooms, which causes your characters (and you) to hallucinate, with one hallucination being a pool of sludge that looks like a hot spring.
