I have no idea if this is a new story or a flash back before Madoka became boss

Considering that both Kyouko and Homura were in maid uniforms, I'm assuming Madoka's mafia has a maid cafe as a front.

Is Madoka daydreaming that Kyouko is a man and Sayaka is his girlfriend?

I wish this series would just have a normal pace already. It's my favorite fan-made concept and it started off really strong, but updates are sparse and they keep changing from one scene to the next.

Maybe it's someone trying to ride on Madoka's rep?

We really have no way of knowing until this gets a translation...

You're mostly on the ball. The pink haired guy is pretending he's the "Kaname" of the Kaname family. It seems that all of Mitakihara knows of it, so he's expecting some 'service' (ie wants a drink for free).
