Apart from the fact that Kanako doesn't seem to have arms (but she has shoulders? Confusing!), which I find mildly disturbing, I find the picture rather cute, actually. Suwako's smile is really heart-warming.

She has arms, she just has them together behind her back to support her torso. You can see a line below the shoulder where her right arm is pressed against the body.

That's one hella-thin arm/shoulder/bone structure she has. I thought it was an armpit.
And the dude who drew this has a thing for drawing girls without certain appendages... or torsos... so it's not too big a stretch to assume she's 'armless.
actually when I see this image, I am also thinking of this.
The similarities are so obvious, IMO, the doujin must have been a strong inspiration at the very least, if this is not outright fanart. Fanart with Touhou crossover of a hentai doujin. Now I've seen everything.
