As someone has said it, Miko is definately a woman, in Touhou at least. She is called the Crown Prince because that sounds cooler I guess

Yesterday, I was taken along by the Chief Maid of the western-style mansion to a place called "karaoke". When we opened the door, there were dozens of totally naked women lying in wait for us, breathing roughly. Within seconds, all my clothing was stripped away. The end of the Chief Maid's soft, heat-filled tongue crawled over the tips of my breasts. With that as a start, the women rushed upon and violated my body like starving beasts. Naturally, I did not resist, for this was what I had hoped for in coming here. At length, ISince arriving here in Gensokyo, I have interacted with all manner of people.
