I'm guessing the idea is that she's so dehydrated, the amount of water she needs to consume would itself be dangerous if not balanced with enough electrolytes.
Not sure why you'd need such a thing for a karakasa, but the fact that she even can get heatstroke means that handsomen has given her a basically human physiology, at least for the sake of the joke.

A basic Oral Rehdydration Solution consists of 30 ml of sugar, 2.5 ml of salt, dissolved into 1 liter of clean water. Sugar is necessary for continued functioning, while salt is used to replenish the sodium in the body. Potassium is also required, since these are the two most common ions exuded in perspiration, so medical ORSs contain potassium chloride as well. A lack of these two ions will disrupt neural signalling throughout the body, leading to cramps, most dangerously in the heart and pulmonary muscles.
If she was given lots of water now, she could get water poisoning, for the reason you mentioned. Carefully metered, even pure water would get her back on her feet, though.
Ironically, Kogasa probably should have been using an umbrella to stay out of direct sunlight.
Note that this is only the second time an umbrella has appeared in the strip, and it is completely Kogasa. The other time had Kogasa carrying a different umbrella. This suggests that in this strip, Kogasa might not have two bodies... she might have fully transformed into the girl form. Which might help explain why she's affected here.
