In the US if you had and could demonstrate safety and competence with an automatic (as in semi or full), a clean record, and could pass a physical examination, many departments held bounties or hired to serve as security during raids for hunting down drug dealers. You still can do this, but the licencing now is absurdly expensive and slow to the point that it is more discouraged in the areas that actually have the biggest narcotics/criminal problems. Private Investigating kind of filled in the gap by reporting information in rather than acting on it.

I wouldn't say that. We have a lot less cartel influence in country than a lot of other countries that don't have active or very small drug enforcement agencies. And what cartels we do have are much more reserved because they know if they stick their head out too much it will get cut off fast. I mean, it is called the "War on Drugs" but no one expects all drugs to be eliminated. That is like asking for all crime to end. While nice, it is unrealistic. Still, it is a heck of a lot better enforcing what you can than letting criminals run rampant. Plus seven degrees of separation is a powerful thing. Those who use narcotics are more likely to have criminal tendencies and a lot of other cases and suspects were found starting with leads on drug trafficking.
You mustn't stop taking this medicine! As in you'll start acting weird if you stop taking it, so you just have to take the medication for the rest of your life. Generally used as an insult, albeit in comedic sense.
