I seriously doubt that. He only ever calls Kal'tsit a hag.
He's literally responding to the several hundred year old vampire who's teasing the two of them. We've also never seen him interact with Warfarin before now, so you don't know that he's never called her a hag.

No he is clearly reacting to a Hag cat that slashed his face because he decided to grab her top and tits in an attempt to pants her because she didn't let him drive the scooter, Warfrin is poking fun at them because damnit, he won't give her his blood so she has to derive some humor from these two being literally oil and water.
This is the first time we've seen Warfarin in this series so we don't know how is her relationship with the doctor here... what we know is that the doctor tends to call Kal'tsit a hag.
I'm inclined to believe that the doctor was still arguing with Kal'tsit when he said that and then Warfarin arrived to poke fun at them.

Guin writes his comics right to left. That makes Warfarin the first person speaking in this image. If Warfarin is first and Doctor is second, comic logic dictates that the second person speaking is replying to the first person speaking. And Doc and Kal are the ones "arriving", as they were late to the meeting.

Why don't we ask the author, then, instead of making a long drawn out non-discussion?

Except the previous strip clearly shows who he's calling a hag, since he ONLY CALLS ONE PERSON A HAG IN THIS COMIC SERIES.

You know who you call a hag? People older than you. Do you know who's probably older than this old man? Kal'tsit and Warfarin. Literally everyone else he regularly interacts with is a child, and the ones that aren't children are still definitely younger than he is. It's almost like it would be really fucking weird if he called any of those precious little babies hags. It's almost like a pissed off doctor has every reason needed to call Warfarin a hag.

Why would he shout Hag at Warfrin for a playful jab and not the cat, he's called hag every other time she's made him mad, that slashed him on the fucking face? Though there is a possibility he's talking to both of them and Guin just used one bubble.

I feel like my IQ has gone down slightly from this comment thread, like holy ****. I mean if you're new to guin_guin then sure, but this is ridiculous. But if you're not, cd_young, then you CLEARLY haven't been paying attention to Guin's style of work and how Guin portrays their relationship.
Its a damn miracle that Warfarin held her shit when Assassin Doc is bleeding from Kal'tsit's nails
Weirdly, I think canonically speaking this shouldn't matter. I can't really tell how Guin is incorporating the game lore into the comic lore, but we've seen that Grandpa Doc's apprentice, who died and left Rhodes Island in Grandpa's care, was probably The Doctor.
