Does it really need one? It makes sense for the base Aqua tag, as there are plenty of characters from other series with that name, but I don't see how Anti-Aqua could be confused for anything else. The search results for "Anti-Aqua" on Google consists almost entirely of KH-related pages.

Yes, alternate forms/costumes are generally meant to match the parent tag in having a qualifier or not. It's less so about the ambiguity and more about the consistency so it's clear from the character tag list. It wouldn't need a qualifier if Aqua didn't have one.

For example, this wouldn't be accepted:

-> Saber (Fate)
--> Saber (Great Amazing King)

However this would:

-> Saber (Fate)
--> Saber (Great Amazing King) (Fate)

(Though I guess there are rare exceptions, like Saber Alter not having a qualifier while the parent tag Artoria Pendragon (Fate) does.)
