Yes, because blood from mouth is a character bleeding from the mouth, and blood in mouth is literally, blood on mouth is a character's OR other's blood AROUND mouth.

We have separation of inside and on for other tags too not just blood so there is some precedent.

Blood on mouth sounds like it can be its own thing, but I don’t know if we should be keeping blood from mouth and blood in mouth separate. It doesn’t seem like those two would be easy to distinguish from each other.
