panty gag and clothes gag are highly subjective, there are few images where a character is clearly gagged with panties and not some other clothing or a cleave gag. These posts can be just as well described with a combination of stuffed gag, panties in mouth, panty mask, cleave gag, etc.
I have considered a new tag like stuffed mouth or something similar but stuffed gag describes the concept fairly well and is a pre-existing tag as old as at least 2018.

cloth gag has become bloat, it seems to be 90% split over three different tags: over the mouth gag, over the nose gag and cleave gag; improvised gag is already quasi redundant and then there's the generic gag as a fallback.

My intro post was already long enough but considering the average content, a stuffed mouth tag would make more sense due to how often it's "accompanied" by other gag tags, this is is a good starting point however to get everything under one roof.

I would say the context makes it pretty obvious.

Four different tags that mean different things vs a single one with a clear purpose. Bad idea. Try to search for panties specifically used as a gag only using those tags. It's impossible.
