You're right, insomnia is not a disease. But she doesn't even have an insomnia!
I mean, normally you sleeps for 6-8 hours, while she wants to sleep for more than 12 hours. Which means she already slept for 12 hours. That would be perfectly normal if you can't sleep aftewards. So....

The joke is: Kaguya wants to sleep more than 12 hours, but get up a particular time, hence the alarm clock; however that kind of alarm clock only lets you set an alarm up to 12 hours, since the alarm goes off when the hour hand reaches the alarm indicator hand. Clocks only show 12 hours at a time.

If you want to sleep more than 12 hours, why would you set up an alarm clock at all? IMO, I don't think she has set up the alarm clock anyway and, in this pic, she's just checking out what time it currently is.
