I don't think there's ever terribly much explicitly said about Rei's origins, but there are certainly several very strong implications all throughout the series that make it pretty clear. Since the offending comment was deleted, here's a spoiler tag'd summary. (Now re-posted because I'm apparently retarded and screwed up the coding so the spoiler tag didn't work the first time.)
The long story short is that Shinji's father (Gendo Ikari) created Rei by using DNA from his recently deceased wife, Yui. As such, Rei is usually believed to be a clone of Yui. I say "usually" because I know there are folks out there who interpret the link between Rei and Yui in different ways. (Ways in which I won't go into unless you really care to hear them.)

You're quite welcome hexentanz. And for the record, Annubis and gladwort and weren't referring to you (though spoiler tags probably wouldn't have hurt with your comment, either..?) There was a comment after yours that basically said what I had said, far more bluntly and without any spoiler tags - that's who they were talking to. It got deleted.
I'm not in the habit of lashing out like that, but what sounded like a key plot element (one that I was previously unaware of) was just blurted out. After all the warnings that have been posted for that very reason, I got really pissed.
It was handled tactfully after the fact, so thanks for that. But please, people--always assume there are others that haven't seen a series, even if it's really old. When in doubt, use the spoiler tags.
