Correcting his mistakes, this is how it happens in real life - but this isn't real life, it's art. In fact, in these cases, it's generally obvious that it's a female with 'male equipment', rather than a male with breasts. Men look different from women; we're curvier and such, and are drawn as such.
Go google a picture of a transsexual and look at the face. It's obvious what they are. It's not the same as it is here.
This is mostly right until the last line. True hermaphrodites in humans are EXCEPTIONALLY RARE, but do happen. The only one in recorded history lives in Minnesota and (last I heard) was working as a female teacher at the University of Minnesota.
She (for that is what she has chosen to live as at the moment) has one testi and one ovary and has in fact been both a father and a mother.
But she looks far more ambiguous in gender than I like.
