The great heroes of civilization, "Insulation" and "Central Heating", I believe, have confined the kotatsu to only Japan... though I do point out that the kotatsu is probably more energy-efficient than heating your whole house, since despite insulation heat can still escape throught eh whole surface, not to mention trying to get fresh air...
