Toyosatomimi was originally a man before being sealed in the mausoleum. It's confirmed in game canon that she is the actual Prince Shotoku, and was male. Tojiko was Shotoku's consort, aka: his lover/mistress.
As for what _cf said, they changed below the waist, means Shotoku became a girl and Tojiko became a ghost.

I have no idea why people keep saying Miko was a man. Her official profile doesn't mention it (her official profile refers to her solely as "she" when explaining her backstory, back when she was emperor), her SoPM profile doesn't mention it, NOWHERE in canon is this mentioned, ffs. One would think this would be a relevant thing to say if it was true.
At MOST there's Sanae ending where she looks at a history book and it says that Shoutoku was a man, but it was a school history book, and the whole thing is pretty much ZUN commenting on how unreliable history can be (like, some historians even claim that Shoutoku didn't exist at all; how can we even know what his true gender was?)

Was she actually male back then or this another case of Saber syndrome ala FSN?
If her official profile said she was an emperor before she was sealed in those ancient times, then she was definitely a male because only males were emperors.

It's not impossible. Though I would wonder why Miko would decide to be remembered as a male insted of female. Unless someone can confirm it. I looked around little bit, but didn't find anything proving that they are a defintive male or female.

Wikipedia recognizes Tipton as a he (trans man), which is the other possiblity: Miko is a transwoman but continued to live as a man [pretty easy to pull that off in her case] for purposes of retaining power. It was canonically noted that while Futo looks basically the same as a shikasen, Miko looks very different for unknown reasons...

How would you call it "transgendering"? As far as I know, transgender people want to change their birth-assigned gender because they don't feel like they were born in the right gender. In Miko's case there is not any proves saying that she hated her old gender? She just changed her appearance to SUIT more with Gensokyo, and this is canon. The fact that the people here keep insist on Miko being trans is canon sicken me, keep your headcanon to yourselves, and this debate is also fucking useless, who the fuck cares if she is trans or not?

This is the only comment that I agree here, wondering why the dislikes, probably those toxic fans always trust in headcanons
