Considering as Futo was the wife of the Emperor, wouldn't she already be old enough to know what romance is? (Explicitly referencing the "Hana yori dango" part.)

The emperor NWSiaCB is talking about is Emperor Sushun, whose wife was Mononobe no Futsuhime (Futo). Meanwhile, Tojiko no Iratsume (Tojiko), consort to Prince Shotoku (Miko), would be Futsuhime's daughter.
So that would mean Futo is Miko's mother-in-law. Have fun with the Ten Desires cast now.
