from what you have just said we can conclude that the shrine maiden is a sort of like "Administrator" or "Game Master" of Gensoukyou...?
The rules allow everyone to ask a rematch and try again, but ZUN commented that the enemies don't do it simply because they don't care. If anything, Reimu and Marisa are actually the tryharding kids that must win at all costs.
As for "Reimu dies = everything follows", there is no real way to know what happens if Reimu dies, because no Hakurei maiden was killed until now giving an answer to it (and if it happened, it means it's not true :P). Personally, i think it's unlikely given Aya's reaction in SSiB, Rinnosuke's comments about how Yukari is basically keeping the Hakurei trapped in Gensokyo, and some other things. Considering also how Gensokyo was initially supposed to be a trap for youkai, it's more likely that it's not true, because if it was that easy they could have just killed the Hakurei maiden/priest of the time to destroy the barrier and be free again.
