Not counting Before-Windows:

EoSD Stage 4: Patchouli and her freaking colorful elements + Koa and those books.

PCB: That stage seems endless. The boss is fun (and takes away half of my life)...

IN: Reimu or Marisa...Reiuma or Marusa....RURUURUGAGAA...I hate that stage.

MoF: "ayayayaya, did the stage just take away 2 of your life?"

SA: Satori, Cat, Stage...

UFO: RELEASE DA MURUSA! (Imo, UFO one is the easiest...still hard though)

TD: YOSHIKA ABUSE!

But for Ten Desire...I feel like Stage 4, 5 or 6 all takes away my life easily.

It's actually a running and deliberate tradition in Touhou.

Personally, stage 4 seems pretty easy for me in most games. Patchouli, Murasa, Seiga+Yoshika are fairly easy to deal with, Satori and Aya are trickier, but still doable. Prismrivers and especially Reimu/Marisa are the real deal, but that I wouldn't call "a tradition".

On the other hand, stage 5... ZOMBIE FAIRIES, WEAVING GHOST PATTERNS, KNIVES, CURVY LASERS. The only exception is probably Reisen, who could easily swap with Reimu/Marisa and the difficulty of both would be suddenly just right.
